2025 Camper Information

To complete your child's registration, the following must be submitted by a legal guardian / parent.  Please complete one form per individual child.

Step 1:  Prior to completing the Camper Information Form, ensure that you have read our Pre-Registration Information.

Step 2:  Download this form (click here). 

Step 3:  Complete this form using a computer and rename file or "Save As":

If you can, please do not send a picture, "Print to PDF", scan, or screenshot of a completed form.  This requires us to re-enter all information manually.

Please return the form (renamed file) as an attachment by email to registration@wannakumbac.com as soon as possible!
